I/We have read the Standards and Requirements for Bar/Bat Mitzvah candidates and I/We agree that the candidate will complete 3 years of Hebrew studies and will remain in Religious School through the year of the candidates Bar/Bat Mitzvah ceremony.

A deposit of $750 must be submitted with this form.

Bar/Bat Mitzvah Fee and Payment Agreement

This fee includes the following:

Bar/Bat Mitzvah Preparation Fee This includes meetings with clergy, up to 26 tutoring sessions, is in addition to all Religious School and Temple Membership fees.

Friday Oneg Shabbat The Bar/Bat Mitzvah family and the Temple co-host a simple Oneg after the Kabbalat Shabbat service (Friday evening before the Bar/Bat Mitzvah) which is prepared for the entire congregation in honor of the Bar/Bat Mitzvah.

Shabbat Morning Booklet Based on information provided by the family the TIOH will produce a booklet for use during the service Shabbat morning that includes a welcome, information about the service, the weekly Torah portion, a personal message from the family and the child, a description of the Mitzvah project. The booklet is informational, personal and a keepsake for your guests.

Candy for the Service will be provided by TIOH - Families are not permitted to bring their own

Security (Saturday Morning) The fee also helps defray the cost of security from approximately 9:00 a.m. to 1:00 p.m. on the morning of the Bar/Bat Mitzvah. Each family contributes toward this Temple cost.

Payment Schedule

• 1st payment of $750 is due upon sending in completed application.

• 2nd payment of 50% of the balance is due 1 year before the Bar/Bat Mitzvah date.

• 3rd payment of remaining balance is due 3 months prior to Bar/Bat Mitzvah date.

Refund Policy

1st payment, $750 application deposit;

• I understand that one-half of the deposit will be returned if my child’s Bar/Bat Mitzvah is cancelled more than one year prior to the date. I understand that the entire deposit will be forfeited if the Bar/Bat Mitzvah is cancelled less than one year prior to the scheduled date.

• 3rd payment of $875 is non-refundable

OPTIONS:

Pulpit flowers (Two arrangements are customary)

Parents of the Bar/Bat Mitzvah usually honor the occasion by providing flowers for the pulpit on the Friday evening preceding the Bar/Bat Mitzvah and for the service itself on Saturday (the same arrangements are used). These can be taken by the family and used for any celebration after the service. The Temple’s Program Coordinator can provide a list of recommended florists. The fee is paid directly to the vendor used by the family.

Upgrade Kiddush and/or Oneg

Families are welcome to upgrade the standard Oneg Shabbat reception and/or the Saturday Kiddush for an additional charge. For further information on upgrades and renting the Temple facilities for an afternoon luncheon and/or an evening party, please contact the Temple’s Program Coordinator at 323-876-8330, ext. 1007.

Tree of Life

We encourage all Bar/Bat Mitzvah families to commemorate their simcha by purchasing a leaf or boulder on the Temple’s Corwin Tree of Life in the Wise Lobby. The price for a leaf is $400 and the price for a boulder is $1500. This is a tax-deductible gift which supports the Temple’s School Scholarship Fund.
